In Overtime, Blue has the ball first, first and goal from the White 10. On the first play the Blue team commits offensive pass interference so after the penalty is assessed it's now 2nd and goal to go from the 25. Next play the White team sacks the Blue QB for a 15 yard loss. It's now 3rd and goal from the 40 yard line. Next play the Blue team is flagged for holding at the LOS. Penalty is accepted so it's now 3rd and goal from the 50. Next play White team sacks the Blue QB again for a 10 yard loss. Now it's 4th and goal from their own 40 yard line. On 4th down Blue heaves a pass to the 10 yard line that falls incomplete, but White is flagged for defensive pass interference.

Where will the ball next be snapped? Whose ball is it? What is the down and distance of the next play? You make the call!

IIRC, in high school defensive PE is a 15 yard penalty with an automatic first down. Spot the ball at the white 45, first down for blue. Play continues.

Since the ball is outside the 20 yard line, I assumed that it was automatically a first and 10.

Yes indeed they do. Under NFHS OT procedures, it's never 1st and 10 in OT. It's always goal to go, no matter the down.
